Stream durable events with cursor replay
SSE event: events contains JSON {data: Event[]} with up to 200 rows. Its id is the last sequence. Deduplicate by sequence and reconnect from the last applied cursor. Keepalive comments arrive after idle reads (30 seconds). A stream-error event contains {code} and closes the stream. Authorization is rechecked before each batch. Initial failures are JSON HTTP errors; failures after streaming starts cannot change HTTP status.
